Entry Checklist and Instruction
Before sending your submission, please confirm that you have completed the entry form and followed all guidance within the form.
When submitting your entry electronically (DoD, PHS, Coast Guard) via this website, please use the following filename format: facilityname.doc
For VHA employees only.When submitting your entry, complete the electronic web registration to signal your intent to submit a proposal.In addition, download and complete the registration form. Submit along with VHA approval signature page and any letters of recommendation (limit three).
Fax your completed submission to 240-314-7332. Your fax transmittal sheet will be verification of receipt.
All entries must be received no later than midnight Eastern Time 31 July 2009.
Important Notes
- Submission limited to 6 pages maximum (12 pt, double spaced, 1" margins)
- Letters of recommendation are optional and are not included in the 6 page limitation. For DoD applicants an electronic section is provided to attach up to 3 letters of recommendation.
- Supervisory and higher level approvals are required for submission
- Six winners will be selected
- All submissions become the property of The Henry M. Jackson Foundation for the Advancement of Military Medicine, Inc. and will not be returned to the sender
- Winners will be contacted in September 2009
- Awards will be presented at a special ceremony at the November 2009 AMSUS meeting
- Public announcements of winners will appear in professional journals following the AMSUS meeting
- Communications or questions regarding the entry process for the Circle of Excellence Award will be accepted via e-mail only
- Prior to, during, or after this award period, neither GlaxoSmithKline nor The Henry M. Jackson Foundation for the Advancement of Military Medicine, Inc. will act as a consultant with any individual or facility regarding a particular practice or program relative to the Circle of Excellence Award program
