Entry Requirements
In order to be considered, each entry must be submitted electronically via this website or via fax for VHA entries and be received no later than midnight Eastern Time 31 July 2009. No e-mail submissions will be accepted.

A completed entry consists of the following elements to be submitted via the form:
- Category of Care Identification
- Contact Information
- Executive Summary
- Program Description, including:
- Identification of Need
- Description of the need for the practice/program, including challenges faced and opportunities presented
- Key Partnerships
- Explanation of key collaborations that contributed to practice/program success (eg, physicians, other healthcare professionals, industry leaders, schools and universities, social services and/or government agencies)
- Creativity and Comprehensiveness
- In-depth overview of practice/program, including information on overall creative approach (ie, resourcefulness, ingenuity, originality) and comprehensiveness
- Measured Impact
- Conclusive account or improvement in patient health/quality of care, including methods used to measure results
