Evaluation and Scoring Criteria
All entries will be evaluated on the following four criteria and scored on a point system as shown below, for a possible total score of 100 points.

Criteria Description
- Identification of Need
- Description of the need for the practice/program, including challenges faced and opportunities presented
- Key Partnerships
- Explanation of key collaborations that contributed to practice/program success (e.g., physicians, other healthcare professionals, industry leaders, schools and universities, social services and/or government agencies)
- Creativity and Comprehensiveness
- In-depth overview of practice/program, including information on overall creative approach (i.e., resourcefulness, ingenuity, originality) and comprehensiveness
- Measured Impact/Outcomes
- Conclusive account of improvement in patient health/quality of care (i.e., improved compliance, access, customer service, etc.) including methods used to measure results
